At Glenway Animal Hospital, our mission is simple: to partner with you to provide thorough surgical care to your patients in a collaborative manner.

We work hard to be a part of your care team in order to provide accessible surgical options for your patients. We prioritize trusting relationships and communication to ensure your practice’s needs are met and you are kept up-to-date with your patients’ care plans.

Surgical Referral Steps

1) Contact

For Pre-planned Surgical Procedures:

Call or email to discuss your patient’s case, or leave dates/times to do so that we may work together to create the best care plan.

If the doctor is not immediately available for a phone consult, leave dates and times that are suitable for the doctor to reach you.

For Same Day Surgery:

Call the office and notify the receptionist that you have a patient that requires same day surgery. The doctor will do her best to be on the phone with you as quickly as possible to discuss the case and suitable arrangements.

2) Referral

Complete this referral form. Upload pertinent medical records and diagnostics performed for our viewing. If preferred, you may also fax us your patient’s information at (513) 662-1010. To download the referral form, click here.

Once the referral paperwork is received, we will reach out to your client to schedule a surgical consult.

If your client decides to move forward with surgery at our office, we will notify you with the date of the procedure.

 

 

3) Surgery Day

Send your client and their pet to 6272 Glenway Ave., Cincinnati, OH 45211.

If you would like updates on your patient throughout the day, we are happy to do so.

Your practice will be sent a complete surgical report as well as a copy of instructions provided to the owner for aftercare and follow-up.
